近年来,全球近视患病率持续攀升,近视在我国呈普遍化、低龄化、严重化趋势,高度近视严重危害青少年眼健康。角膜塑形镜是目前临床延缓近视进展的有效手段之一,但其确切的作用机制目前尚未完全阐明。文章围绕学界提出的多种假说,综述了周边视网膜离焦、脉络膜厚度与血流、高阶像差、调节及生物力学等因素在角膜塑形镜近视防控中发挥的作用。其中,角膜塑形镜通过逆几何设计使周边视网膜形成近视性离焦,是其延缓眼轴增长的核心机制; 戴镜后脉络膜增厚、血流灌注增加,可改善视网膜外层营养供应并抑制巩膜重塑; 球差、彗差等高阶像差增加,可扩大景深、减少调节滞后; 调节功能的改善及角膜生物力学的改变亦协同发挥作用。各机制之间相互关联、层层递进,共同抑制眼轴增长。文章旨在为深入理解角膜塑形镜的作用机制提供理论依据,并为角膜塑形镜的个体化验配、镜片优化设计及后续近视防控研究提供临床参考。

In recent years, the global prevalence of myopia has continued to rise. In China, myopia shows a trend toward widespread prevalence, earlier onset, and greater severity, and high myopia poses a serious threat to adolescent ocular health. Orthokeratology is currently one of the most effective clinical interventions for slowing myopia progression, yet its exact mechanisms of action have not been fully elucidated. Focusing on the various hypotheses proposed in the literature, this review summarizes the role of peripheral retinal defocus, choroidal thickness and blood flow, higher-order aberrations, accommodation, and biomechanics in myopia control with orthokeratology lenses. Specifically, the induction of peripheral myopic defocus through the reverse-geometry design of orthokeratology lenses constitutes the core mechanism by which axial elongation is slowed. After lens wear, choroidal thickening and increased blood perfusion improve the nutritional supply to the outer retina and inhibit scleral remodeling. Increases in higher-order aberrations, such as spherical aberration and coma, expand the depth of focus and reduce accommodative lag. Improvements in accommodative function and alterations in corneal biomechanics also act synergistically. These mechanisms are interrelated and act in a cascading manner to jointly inhibit axial elongation. This review aims to provide a theoretical basis for a deeper understanding of the mechanisms of orthokeratology lenses and to offer clinical references for individualized lens fitting, optimized lens design, and future research on myopia prevention and control.
