甲状腺相关眼病(TAO)是与自身免疫性甲状腺疾病密切相关的器官特异性自身免疫病,以眼眶成纤维细胞活化、糖胺聚糖聚集及眼外肌纤维化为典型病理特征,严重影响患者视觉功能与生活质量。自身免疫应答失衡是TAO核心发病机制,其中辅助性T细胞17(Th17)与调节性T细胞(Treg)的平衡紊乱在疾病进展中起关键作用。文章综述Th17/Treg细胞失衡在TAO中的研究进展,先概述TAO免疫病理机制,明确促甲状腺素激素受体、胰岛素样生长因子1受体及多种眼眶自身抗原的作用; 再阐述Th17与Treg细胞的生物学特性及功能平衡机制,涵盖Th17细胞分化关键转录因子维甲酸相关核孤儿受体γt、Treg细胞核心功能分子叉头状转录因子P3的调控作用; 进一步总结TAO患者中Th17/Treg失衡的临床证据,以及Th17细胞通过分泌白细胞介素-17A等促炎因子驱动眶内炎症、Treg细胞因数量减少与功能缺陷导致免疫抑制失衡的具体作用机制; 最后梳理靶向该失衡的治疗策略,包括细胞因子拮抗、表观遗传调控、免疫检查点干预等方向的研究成果,并指出当前研究不足与未来方向。文章旨在为TAO免疫病理机制研究及临床精准治疗提供参考。

Thyroid-associated ophthalmopathy(TAO)is an organ-specific autoimmune disease closely associated with autoimmune thyroid diseases. It is pathologically characterized by orbital fibroblast activation, glycosaminoglycan accumulation and extraocular muscle fibrosis, which seriously impair patients' visual function and quality of life. Autoimmune response imbalance constitutes the core pathogenesis of TAO, among which the disturbance of T helper 17 cell(Th17)and regulatory T cell(Treg)homeostasis plays a pivotal role in disease progression. This paper reviews the research progress of Th17/Treg cell imbalance in TAO. It firstly summarizes the immunopathological mechanism of TAO and clarifies the roles of thyroid-stimulating hormone receptor, insulin-like growth factor-1 receptor and various orbital autoantigens. Then it elaborates the biological characteristics and functional balance mechanism of Th17 and Treg cells, covering the regulatory roles of retinoic acid-related orphan receptor γ t, the key transcription factor for Th17 cell differentiation, and forkhead box protein P3, the core functional molecule of Treg cells. Furthermore, it concludes the clinical evidence of Th17/Treg imbalance in TAO patients, as well as the specific mechanism by which Th17 cells drive intraorbital inflammation via secreting pro-inflammatory factors such as interleukin-17A, and the immune dysregulation caused by the reduced number and functional defects of Treg cells. Finally, it sorts out therapeutic strategies targeting this imbalance, including research advances in cytokine antagonism, epigenetic regulation, immune checkpoint intervention and other directions, and points out the deficiencies and future prospects of current studies. This review aims to provide references for the research on immunopathological mechanisms and precise clinical treatment of TAO.
