眼表需维持稳态以抵御持续环境刺激,而蒸发增加是蒸发过强型干眼病程进展的重要起始环节。泪膜脂质层的组成、分布与功能决定其抗蒸发能力; 睑板腺功能障碍、脂质层异常、瞬目异常、黏蛋白不足及上皮屏障受损等多因素叠加,削弱蒸发屏障,继而诱发高渗应激和眼表炎症,激活神经-免疫通路,形成自我强化循环并促进病程迁延。西北地区低湿、少雨、风大、风沙频繁且蒸发量高的环境特征可进一步加重上述过程。文章结合现代眼表病理机制与“西北燥证”理论,综述蒸发过强型干眼的发病机制及干燥环境对其的影响,以期为我国西北地区干眼的风险识别与干预提供参考。

The ocular surface must maintain homeostasis to withstand continuous environmental stimuli, while increased tear evaporation represents a critical initiating event in the progression of evaporative dry eye disease. The composition, distribution, and function of the tear film lipid layer determine its anti-evaporative capacity. Multiple factors, including meibomian gland dysfunction, abnormalities of the lipid layer, blinking disorders, mucin deficiency, and epithelial barrier impairment, collectively weaken the evaporative barrier, subsequently inducing hyperosmotic stress and ocular surface inflammation, activating neuro-immune pathways, and thereby establishing a self-perpetuating cycle and contributing to disease chronicity. The environmental characteristics of northwestern regions, including low humidity, limited rainfall,strong winds, frequent windblown dust, and high evaporation rates, may further exacerbate these processes. This review integrates modern ocular surface pathological mechanisms with the theory of “Northwest dryness syndrome” to summarize the pathogenesis of evaporative dry eye disease and the effects of dry environmental exposure, aiming to provide a reference for risk assessment and intervention strategies for dry eye in Northwest China.
