[摘要]
目的:分析量规、IOL Master及Oculyzer眼前节分析系统测量角膜水平直径(WTW)的相关性和一致性。
方法:回顾性分析2018-07/2019-01于我院行有晶状体眼人工晶状体(ICL)植入术的高度近视患者41例80眼,术前分别用量规、IOL Master及Oculyzer眼前节分析系统测量WTW,分析其相关性和一致性。
结果:本组患者术前采用量规、IOL Master及Oculyzer眼前节分析系统测量WTW的结果分别为11.73±0.40、12.03±0.46、11.54±0.39mm。量规与IOL Master、量规与Oculyzer、IOL Master与Oculyzer测量结果均呈正相关(r=0.809、0.826、0.785,均P<0.01),Bland-Altman分析95%一致性界限(LoA)分别为(-0.84,0.22)、(-0.27,0.64)、(-0.07,1.05)mm,一致性较差。
结论:三种仪器测量WTW,IOL Master测量值最大,Oculyzer测量值最小,三种测量方法一致性较差,在临床上不可相互替代,但三者之间相关性较好,临床应用时可根据IOL Master、Oculyzer与量规之间的差值矫正测量值。

AIM: To analyze the correlation and agreement of corneal horizontal diameter(WTW)measured with caliper, IOL Master and Oculyzer anterior segment analysis system.
METHODS: Totally 41 patients with 80 eyes who underwent ICL V4c implantation in our hospital from July 2018 to January 2019 were enrolled. Compare the correlation and agreement of preoperative WTW measured with caliper, IOL Master and Oculyzer anterior segment analysis system respectively.
RESULTS: The mean WTW value measured with caliper, IOL Master and Oculyzer anterior segment analysis system were: 11.73±0.40, 12.03±0.46, 11.54±0.39mm respectively, the value of caliper and IOL Master, caliper and Oculyzer, IOL Master and Oculyzer were highly correlated(r=0.809, 0.826, 0.785, all P<0.01), and the 95% LoA of Bland-Altman analysis were(-0.84, 0.22),(-0.27, 0.64),(-0.07, 1.05)mm respectively.
CONCLUSION: Among the three measuring instruments, IOL Master has the largest measurement value and Oculyzer is the smallest. The three measurement methods have poor consistency and cannot be replaced in clinical application. Whereas, their measurements are highly correlated. In clinical application, the measurements of IOL Master and Oculyzer can be corrected based on the difference with caliper.
