[摘要]
目的:探讨传统泪膜测量方法和非侵入性泪膜功能检测方法的相关性、一致性及舒适性。
方法:临床病例回顾性研究。选取101例101眼欲行准分子激光角膜屈光手术患者,应用Oculus眼表分析仪测量非侵入性泪膜破裂时间(NIBUT)、下泪河高度(LTMH); SchimerⅠ试验(SⅠt)测量基础泪液分泌、侵入性泪膜破裂时间(fl-BUT)。分别对首检眼的NIBUT与fl-BUT进行相关性、一致性分析,对LTMH与SⅠt进行相关分析。采用视觉模拟评分法(visual analong scale,VAS)对两种检查过程的舒适度进行评价。
结果:LTMH与SⅠt呈正相关(rs= 0.346,P=0.001)、NIBUT与fl-BUT呈正相关(rs = 0.393, P=0.001)。NIBUT与fl-BUT的Bland-Altman图 95%一致性界限范围-9.62~14.18。NIBUT检查与fl-BUT检查舒适度评分比较,差异有统计学意义(z=-2.324,P=0.020); LTMH检查与SⅠt检查舒适度评分比较,差异有统计学意义(z=-8.845,P=0.001)。
结论:Oculus 眼表综合分析仪测量的NIBUT、LTMH值客观有效,LTMH与SⅠt呈良好的正相关,NIBUT与fl-BUT、SⅠt与LTMH存在正相关,且检查过程较传统方法舒适,能够有效评估角膜屈光手术术前泪膜功能。

AIM: To explore correlation, consistency and comfort between traditional tear film examination methods and Oculus Keratograph.
METHODS: A retrospective study. Totally 101 cases(101 eyes)were diagnosed myopia and then accepted LASEK(laser epithelial keratomileusis). Non-invasive tear film break-up time(NIBUT), lower tear meniscus height(LTMH)were measured with Oculus Keratograph, fluorescein tear film break-up time(fl-BUT)and Schimer Ⅰ test(SⅠt)were performed on all cases. The correlations analysis between NIBUT and fl-BUT, LTMH and SⅠt were performed by Spearman rank correlation, consistency check between NIBUT and fl-BUT by Bland-Altman analysis. Visual analogue scale(VAS)was applied on evaluating the comfort of two kinds of examination methods.
RESULTS: LTMH and SⅠt showed positive correlation(rs=0.346, P=0.001). NIBUT and fl-BUT showed positive correlation(rs=0.393, P=0.001), 95% consistency limits range -9.62 to 14.18 in Bland-Altman Figure. There was significant difference between VAS of NIBUT and VAS of fl-BUT(z=-2.324, P=0.020). There was significant difference between VAS of LTMH and VAS of SⅠt(z=-8.845,P=0.001).
CONCLUSION: Oculus Keratograph can objectively measure NIBUT and LTMH, and was more comfortable than traditional tear film examination methods. It can effectively assess tear film function before corneal refractive surgery.
